General Expectations

Medical Care

Our goal is to provide the best medical care possible and will take every step to ensure we do this with time and costs in mind.

Convenience

We look to provide convenience through being able to contact me through text, cell, email and same day appointments. While we want to provide as much convenience as possible, I won’t sacrifice the best care possible for sake of convenience

Mutual respect

Any good relationship is based on mutual respect. I ask that you are respectful to my staff. They are doing the job I have asked them to do. If there is a problem or misunderstanding with this, please address it with me.

Appointments

We ask that you arrive 10 minutes prior to your scheduled appointment time. This gives the front desk and medical assistants the time they need to verify your information, correct any updates, take your vitals and ensure you are ready for an on-time appointment with the doctor. I will make every accommodation to see you if you are late but will not infringe upon another patient’s scheduled appointment time. If you are too late, you will be offered a later appointment slot that day, if available.

Same Day

These appointment slots are saved every day to be sure you can be seen within 24 hours. We allocate 15-20 minutes for these slots. I am happy to address more than one issue at these appointments, but for issues requiring more time, I will ask we schedule a separate appointment.

Follow up

These slots are scheduled for 20-40 minutes. I am happy to address multiple complaints on these visits. If you have multiple issues we need to address, please create a list in the order you wish to have them addressed.

Physical

Physicals are generally a covered service on your insurance. We allocate up to 40 minutes for these appointments. It is best to come in fasting, in case we need to do any blood work. A physical generally consists of a review of your health prevention, medications, surgical and family history and treatment for ongoing medical issues. I am happy to address any new concerns you may have, time allowing. I would encourage you to take advantage of this opportunity, as it is a covered service on your insurance that you pay for.

Courtesy Visits

We do offer courtesy visits to friends and family who are visiting from out of town. These visits are meant to avoid costly and time-consuming visits to urgent care centers or emergency departments. These visits are billed to the patient’s insurance. They are not intended for continuity care.

Dependent Care

Dependents who are under 26 years of age and on your insurance fall under the MDVIP umbrella. We offer all the same services with the exclusion of the Annual Wellness Exam.

Annual Wellness

Pre-Wellness

The pre-wellness exam involves the labs and testing. It begins with a blood draw that is sent to the Cleveland Heart lab. In preparation for these labs, we ask that you avoid exercise and seafood within 24 hours, and make sure you are fasting from 10pm the night before. After the blood draw, the remainder of the tests will be performed. You typically do not see a physician on this visit. Please plan an hour.

Annual Wellness

The annual wellness generally lasts 45-60 minutes and includes a review of your health history, an update on any preventative measures, review of all the testing we do and then addressing any issues you may have otherwise.

Afterhours Appointments

I will schedule appointments after hours or weekends on a selected basis. These appointments are generally 15-20 minutes long. I ask that you keep these appointments as focused as possible, as I usually have time planned to spend with my family.

Blood Draws/Nurse Visits

These appointments are just with one of the medical assistants. They are meant to give vaccines, draw blood or teach different clinical things. You will not see the doctor at these appointments. We are happy to draw blood ordered by another physician, but need the order in advance to ensure we have the proper ‘tubes’ to draw whatever is needed. Labs ordered by other physicians do not come back to our office, and go to the ordering specialist. We will not draw labs to diagnose or monitor care performed by another physician.

Insurance

MDVIP works in conjunction with your insurance to provide you with care. Office visits will be billed per your policy guidelines and our contractual guidelines with insurance. We will make every effort to keep things cost-effective but have contractual and regulatory obligations we are bound to. Prescriptions

Medical Centers of Excellence

Through MDVIP, we can make referrals to different tertiary centers, including the Mayo Clinic here in Jacksonville. These referrals are coordinated by MDVIP. Often times, we can get you in with specialists who are otherwise unavailable.

Specialist referrals

Being independent, our network of referral sources is based on who gives us the best medical care and customer service. While this sometimes changes, we have a long relationship with many of the specialists we refer to. We base our relationships on the quality of care they give, not the network they belong to.

Travel

If you are planning a trip, I have a ‘travel kit’ I have designed to take care of most medical issues that may be encountered while traveling. Additionally, if traveling overseas, we can review the CDC Traveller’s website to make sure there are no precautions for the place you are traveling to. I recommend sitting down and having a” travel consult” at least 2 weeks prior to traveling. These can be done by telehealth as well.

Controlled substances are not always allowed to be filled out of state and some cannot be prescribed across state lines. If taking these medications and planning to travel, please touch base before you leave so we can make accommodations for this.

Wellness Center

All MDVIP Members have access to our wellness center for a nominal monthly fee (those joining prior to June 2023 do not pay) of $9 per month. Ethos project wellness program

Our wellness program is a yearlong program. It starts with biometric testing and individualized design of nutritional and exercise programming, followed by interval check-ups throughout the year to keep you on track. It includes use of the wellness center and personalized coaching through the Bridge app.

Dietician

We have a registered dietician on staff who can design nutritional plans for you, in addition to offering coaching throughout the year.

For those interested in personal training, we offer packages through our wellness director, Marc Gonzalez.

Galleri is a cancer screening blood work using liquid-based DNA technology to test for 51 different types of cancer. For more information, please email me and I will send you more information. The cost through MDVIP is $749 down from $949.

This is the gold standard for measuring cardiorespiratory fitness. It can be used to measure how fit you are and also to help you train for athletic pursuits. It is also one of the best ways to predict your future health.

RMR, or resting metabolic rate, is the best way to determine your calorie needs for nutritional planning. Based on this, we can design meal plans to get you to your goals, be it lose weight, get off medication or gain muscle.

DEXA Body composition

Doing a body composition with DEXA is the most accurate way to measure your fat and muscle, and where you put it. It’s a great way to see where you are at in your health journey, and an even better way to visually track your gains.

PRP

PRP, or platelet-rich plasma, is an injection therapy beneficial for a variety of issues that fail traditional therapy. This would include tendon injuries, ligament injuries and in some cases arthritic joints.
