We want to make sure you get the most from your visit with us at the Center for Health and Sports Medicine. Prior to your appointment, we spend time preparing resources and reviewing your chart, but by following the suggestions below, we can work together to gain the most from the time given for your appointment.
One
Make sure you arrive 10 minutes prior to your appointment. This allows us to take care of any changes on your account, get you checked in and have the staff call you back and get your vitals so you are ready to see your provider at the scheduled appointment time.
Two
Please bring a list of medications, supplements, vitamins and alternative medications you may be taking. Include the dose and frequency. If easier, bring the bottles with you to your appointment.
Three
Write a list of concerns in the priority you wish to address with your provider. Please understand that problems vary in complexity and the time it takes to address them, so we may not be able to address all your concerns at a single visit.
Four
Research for yourself before your appointment. While the internet can give you varied information, we encourage you to educate yourself as much as you can. We will make recommendations of websites that are validated sources of information for you.
Five
Make sure we have your correct cell phone number and address, as the reminders we send for appointments come through these forms of communication. You are welcome to opt out of these, but are ultimately responsible for being at the appointment on time.
Six
Bring a photo ID, an active insurance card, and a method of payment to each appointment, regardless if we have these on file. Failure to do so may cause a delay in your appointment.
